This referral form is set up for an agency referrer to ask WorkLife Wellness Lab to reach out to the client first to establish fit, or to make a direct referral with authorization attached.
To advise efficiently, please include any information such as recent resume, cover letter, IPEs, ROIs, and specific client needs and goals, so we can efficiently evaluate the agency referral.
WHAT HAPPENS ONCE THE FORM IS SUBMITTED?
Once you submit the form, we will request the client set up an appointment with by email and text. Please inform your client that this will occur, and have them be on the look out for a message from us. Our goal is to have a message out to you in 1-3 business days that we are moving forward with the contact. However, it may take upto 14 days to complete the contact, assuming everyone responds and schedules accommodate. We attempt to contact by email and text 3x over a 10 day period.
WHAT KIND OF COMMUNICATION CAN YOU EXPECT FROM US?
We are using a tool called jotform to track your referral submission. So notifications will come from there.
You will get a confirmation email stating your referral has been submitted (received immediately), your referral has been approved or declined (within 3 days of the submission) , your referral has been accepted, or rejected (within 14-17 days of the initial request).
HANDLING AUTHORIZATIONS AND BILLING
Authorizations can be submitted in two ways, at the time of the referral by stating you are attaching an authorization, and then attaching the auth via the file upload box provided. Or you can send all auths for new or recurring services to billing@wlwlab.com
If you have questions about the auth billing attached to the auth, please email billing@wlwlab.com, please do not CC the coach or managing biller directly on these inquiries, it will duplicate the message and may delay response time.
Yes, there is a spot in the form where you can write a narrative in lieu of attaching forms.
Keep in mind that if those forms are necessary for us to move forward with an introduction call, or move forward with establishing services because we have already agreed to provide services, then not attaching the documents to this form will delay services.

This form is for the referring agency to complete if you are trying to coordinate a meet and greet between parties, want us to reach out directly, or have authorized services because we have agreed to move forward for the first time. If you want to have the client initiate contact, have them visit our site, and click on the for you menu for information regarding services and how to connect.
The more complete the referral form is the faster we can coordinate scheduling. If you have not authorized the services requested in the referral form, we will be reaching out regarding the decision and next steps if any. For referrals that are received with authorizations in place, we will process those requests within 72 business hours, and the scheduler will be reaching out as specified in the form, to coordinate scheduling. You will submit initial and future authorizations to billing@wlwlab.com. From there our billing team will update our records and reach out to you directly if there are additional questions.
